Referring patients to the service
A referral form for the Get Healthy Service is available for Health Professionals and General Practitioners. All you need to do is complete the Health
Professional Referral Form or the General Practice Referral Form and email or fax it to the Get Healthy Service on the details below. The GP Referral Form includes the medical clearance process. However, the Health Professional Referral Form does not include the medical clearance process and medical clearance may still be required from a General Practitioner.
General Practitioners are being asked to determine the suitability of their patients to participate in the ongoing coaching component of the Service by completing a medical clearance for them. The medical clearance form will be taken by the potential participant to their General Practitioner and the General Practitioner will be asked to determine whether their patient is:
The General Practitioner can then notify the Service that their patient is cleared to participate.
General Practitioners are also being encouraged to refer any client they think would benefit, to the Get Healthy Service. All you need to do is complete the General Practitioner Referral Form and email or fax it to the Get Healthy Service on the details below.
The GP Referral Form has been included on the latest software update for Medical Director. A template for Best Practice has been developed by Networking Health NSW (formerly General Practice NSW) and is available on the Networking Health NSW website.
With the permission of the participant, the General Practitioner will be kept informed of their patient’s progress with the provision of regular reports from the Service.
